Includes five circular shaped paper parts that bend and fold to make up the nine holes (1 & 9; 2 & 8; 3 & 7; 4 & 6; 5 & 5); plus an instructional card with five hazards. Everything printed in red and shades of green over white paper. Paper parts housed in the original red and green printed folder with various hazards printed on it. Folder measures 7x8¼" (parts slightly smaller).

Fun golf practice aid for putting in your home. Perforated portions on parts unopened, so never used. Clean and bright overall, looks like new. Scarce with no example found while searching the internet or other references.
