Printed form. 4 pages, a single folded sheet. 29x20 cm (11½x7¾"). 

A early United States income tax form - the first personal income tax was instituted in 1861, at the beginning of the Civil War. Space is given for listing salary, rental income, dividends, profits from sale of gold, etc., and losses from same. There is a standard $1,000 personal deduction, "Exempt by law." Though the form clearly states the penalties invoked by failure to pay the tax, the recipient of the form, one A. Castillo of New Orleans, evidently chose to risk the consequences, and did not fill out the form.

Invoices are sent out the day following the auction and include a minimum $10 handling fee plus actual shipping and insurance charges.
